To be come a member, please complete the Membership Application and mail it along with your annual membership dues to: FOFA, PO Box 2022, Forks, WA 98331
Membership dues fund our spay/neuter program, for emergency medical care for injured animals, and various supplies associated in providing assistance to animals.
