Annual Membership
Individual: $20 annually
Business: $40 annually

To be come a member, please complete the Membership Application and mail it along with your annual membership dues to: FOFA, PO Box 2022, Forks, WA 98331
Membership dues are for a calendar year.
Membership dues fund our spay/neuter program, for emergency medical care for injured animals, and various supplies associated in providing assistance to many animals in the West End of Clallam and Jefferson Counties of Western Washington State.
